Primary Care Network

Dear All,

I am sure many of you are aware of a new GP contract that will be in place for the next 5 years.  This represents one of the biggest changes in the shape of general practice for decades.

Essentially, we will be expected to join up with neighbouring practices under a new contract called the Primary Care Network Directed Enhanced Service and to start delivering services together.  In addition, we will be expected to work far more closely with our community services and hospitals.

There may also be a difference in the way we deliver some of our locally commissioned services, for example, reviews of medications for patients with complex care needs, provision of home services etc.  This will evolve and develop over the next few years.

We hope these changes will be an effective way to work at scale in the future to improve patient care and we look forward to working with everyone in our PCN practices.
